A defined and repeatable system for analysing risk in the provision of medical services. The system involves the assessment of actual or potential failure modes and determining the severity of the failure, the rate at which it will occur and the ability of the medical service provider to detect the failure. For each of the three categories (severity, occurence and detection), the user is provided guidance on how to quantify the dimession on a scale of one to ten. The Medical Services FMEA then permits the user to compare the total risk present for each failure mode against the others present. This comparison permits the user to identify the failure modes most in need of management action.
